Doctor Julian Rush is a psychiatrist at Arkham State Hospital and the therapist of Sofia Falcone, with whom he is in love. He later joined her in building her criminal empire as her advisor.

Even after Sofia Falcone was released from Arkham State Hospital after completing her rehabilitation, Doctor Rush continued to see her to help her work through her PTSD that she suffered in her past and at Arkham. After the death of Sofia's brother Alberto, Doctor Rush had a session with her where she suffers hallucinations and a panic attack. Rush calms her down by telling her to take deep breaths and look around the room, reminding her that she is safe. Sofia leaves and Doctor Rush asks what she saw, still wanting to help her, while she remarks that she is not safe, only "home."[1]

At first glance Julian appears to be a simple doctor, committed to the Hippocratic Oath and willing to help Sofia. According to him, he does so because he needs to redeem himself for not believing her when she was locked up in Arkham.
However, it is eventually revealed that Julian is quite mentally disturbed himself: he seems to have an obsession with fear and the idea of feeling free even if he has to commit atrocious acts. According to Sofia, Julian enjoys seeing a person break down emotionally. One constant in Julian's behavior is his fascination, some would say obsession, with Sofia and desire to be with her forever.
Julian appears to have sadomasochistic sexual tendencies.
