Milos Grapa was the Consigliere of the Falcone Crime Family, and the former bodyguard of the late Carmine Falcone. In the aftermath of the assassination of Carmine, and then the unexpected disappearance of Alberto Falcone, Grapa would assist Luca Falcone with taking over the family, while they dealt with escalating tensions with the Maroni Crime Family, who had taken responsibility for Alberto's death, as well as the return of Sofia Falcone from Arkham State Hospital. All these tensions would finally hit a boiling point when Sofia had then made the decision to murder everyone in the Falcone Crime Family, including Grapa.

At some point, Milos joined the Falcone Crime Family, and started out as Carmine Falcone's bodyguard, as well as a hitman, being notable for having killed numerous people. He eventually rose up to become the Family Consigliere, due to his wise and calculating nature.
